Estudio de variabilidad de facturación a pacientes operados a colecistectomÃa por videolaparoscopÃa en el Hospital ClÃnico de la Universidad de Chile

Medical decisions often try to be based on science, but in the mojority of decisions this base doesn't exist. Furthermore it is known that very different medical procedures can produce the same results and this is associated with highly variable resource consumption. We propase to determine and quantify the differences existing between patient's charges of an operation for asymptomatic cholelitiasis, without complication using laparoscopic cholecistectomy. We analyzed hospital bilis of 224 patients, randomly chosen from a total number of 1022 patients operated on for cholelitiasis using laparoscopic cholecistectomy during 2000 at the Universidad de Chile Clinic Hospital.
Patients who presentad any medical or surgical complication were excluded in order to make the
sample homogeneous.
We found significant diff erences in patient's bilis in this group, with sorne pathology so/ved with sorne technique, without complications. According to study design, surgeons preferences account for the differences in charges. These preferences are relatad to laboratory tests, drugs, materials. Results suggest that it is necessary to establish protocols and clinical guidelines for more prevalent pathologies.
